Constructed ratings

Constructed ratings are calculated on the basis of all matches counting
for ABC-common, Type 1, and Singleton. Here we count full (best of 3)
matches.
   In those formats we still have very few matches played, so the rankings
are statistically not terribly solid. Only those who have played in at
least one of the constructed formats are included here.
